As part of the documentation necessary for our school to receive Title I funding, we are required to ask all parents to take part in a Parental/Community Involvement Survey. A link is provided here for your convenience to complete the survey. It should take less than 5 minutes to answer the ten questions. Your help in this matter is greatly appreciated.
USD 511 Parent Survey of Title I students
If Mrs. Smith works with your child on a weekly basis, we ask that you also complete this second survey. This, too, is part of the documentation that we must provide in order to continue receiving Title I funding. Thank you for your support in this matter.
